North Central Health District Offers Chance to Help Your Community and Win $100

North Central Health District (NCHD) is offering residents of the 13-county district the chance to win a $100 gift card by assisting with a Community Health Assessment.

To participate, residents must complete a short Community Health Assessment Survey at (SURVEY NOW CLOSED) before Nov. 8, 11:59 p.m. Each resident that completes the survey will be entered into a random drawing with other residents of their county. Each county will have a winner selected to receive a $100 gift card.

The Community Health Assessment Survey is about more than winning a prize. The survey gives Middle Georgia residents an opportunity to tell their local public health providers about what health issues are most important to the community. The results of this questionnaire will help guide the efforts of NCHD by providing foundations for planning public health initiatives.

“We want to gather as much information from residents as possible when it comes to planning our actions,” said Amber Erickson, Director of Epidemiology, Community Assessment and Research Initiatives. “The more feedback we receive from a community perspective, the better we can serve our district’s public health needs. Our staff would like to have input from as many people as possible so we can get a clear picture of where we need to focus our efforts in the coming years.”

NCHD’s Community Assessment Program focuses on systematically collecting, analyzing, interpreting data that is used to design control and prevention measures, to evaluate the effectiveness of public health interventions, and to improve services to populations at greatest risk. The latest Community Health Assessments are available for all 13 NCHD counties online at NCHD52.org/Community.
