The HOPE Center is a comprehensive, Ryan White-supported health clinic for individuals living with HIV/AIDS. We are dedicated to meet your medical needs and assist you in managing your well-being. The HOPE Center is a service of North Central Health District, covering its 13-county jurisdiction. We are located at:

If you are a HOPE Center client and are experiencing a medical issue after hours or on a weekend, please contact your primary care provider.  If it is an emergency please dial 911 or go to your nearest hospital emergency room for treatment:

Medical Services

Our goal is to help every client achieve HIV treatment success and a healthy life. We believe in client-centered health care. We’ll work with you to help you sustain a healthy immune system and reduce HIV to a manageable level.

Comprehensive Services

When you enroll at HOPE Center, you’ll immediately have access to the most comprehensive HIV services in middle Georgia. You’ll receive complete treatment of HIV and outstanding primary care for the treatment of other health conditions. Should you need specialist care, we’ll even provide outside referrals.

Personalized Care

Everyone has unique needs. Here, you’ll have a medical team that works closely with you to create the treatment plan that’s best for you. They will assess your needs and help you get the care and support you need to achieve treatment success.

A Medical Home

A HOPE Center, you have access to an array of medical and social services including medical care, dental care, mental health counseling, housing assistance, nutrition services, transportation and case management.

 

Case Management & Social Services

Nothing is more important for people with HIV than having access to quality health care. If you have difficulty accessing the health care you need or receiving the financial benefits that are rightfully yours, you can lean on our case management team and their expert knowledge in navigating the health care system. Their top priority is to help you get enrolled in quality HIV health care and to ensure that your health care benefits are secure.

HOPE Center social services help eliminate serious barriers to successful HIV treatment. Homelessness, poor nutrition, discrimination, benefit denials and poverty are common problems that often overwhelm clients. These problems can become so severe that it’s impossible for patients to give the attention that’s needed to their HIV treatment plans.

If you experience any of these problems, HOPE Center social services are here to help you strategize and develop solutions that focus on your health and well-being.

Comprehensive Needs Assessment

Your social work/nurse case manager will meet with you to do a full assessment of you needs including:

  • Medical, dental and behavioral health needs
  • Nutritional, housing and other social service needs
  • Financial needs
  • Your eligibility for government benefits
  • Resources or referrals for family and child needs

Care Plan Creation

Based on the results of your comprehensive needs assessment, the social work case manager will work with you to create a care plan personalized for you. This plan will address your needs and resolve issues you may have.

Medical Care Enrollment

If you aren’t enrolled in medical care, your social work case manager will provide you with information on HOPE Center services, as well as a range of services available to you through other medical care providers. Your case manager will coordinate your enrollment as a client with the provider of your choice.

Advocacy For Your Benefits

Our social work case managers will work to secure your benefits from Social Security, Medical Assistance/Medicaid, Medicare, commercial insurers and other benefits that are rightfully yours.

Enrollment in Other Health Services

Based on your comprehensive needs assessment, your social work case manager will work with you to coordinate your enrollment in medical, dental, pharmacy and oral health care and social services at HOPE Center and other community service providers.

Peer Support

HOPE Center employs a team of individuals who are living with HIV and who can assist you in your journey in understanding your HIV diagnosis, as well as how to maintain your health. They are available to support youwhen you struggle with treatment and care, and can provide support and guidance on how to talk with friends and family about living with HIV.

Support Groups -Coming Soon!

HOPE Center’s behavioral health staff works to provide multiple in-house supprt group services. In addition, outside providers may be contracted to facilitate groups.

Dental Services - Coming Soon!

As a client at HOPE Center, you’ll receive outstanding preventative care with regular teeth cleanings and oral hygiene education. Our expert dental staff understand the challenges you may face in receiving dental care, including trying to find an experienced dentist or not having a way to pay for dental care. You won’t experience those challenges when you receive dental care at HOPE Center.

Comprehensive dental care is essential for HIV patients. HIV medication may cause oral conditions that require frequent dental visits. Oral symptoms of advanced HIV can be identified early by our HIV-experienced dentists and then quickly treated by your health care provider. Complete restorative care may also be provided if you need fillings, crowns, bridges or dentures. Note that annual cost limits apply to restorative care services.

Behavioral Health Services

If you experience stress, anxiety, depression addiction or any other mental health challenge, HOPE Center will do its best to assist you. Our behavioral health and wellness services are available to help you overcome these mental health challenges. Whether you are newly diagnosed with HIV or have lived many years with HIV, our goal is to help you be mentally and emotionally healthy.

Through HOPE Center’s behavioral health and wellness services, you can receive screening and diagnostic assessments of mental health conditions you may have. You’ll have access to counseling services and may receive referrals for psychiatric evaluation, alcohol and other drug treatment services. You may also participate in one of our many groups or join a wellness program.

HOPE Center maintains onsite staff and contracted counselors and therapists that are experienced mental health professionals. We are dedicated to your well-being and will always assure that you are treated with dignity and respect in a confidential and supportive environment.

COMPASS Center Pharmacy and Contracted Services

We strive to make our access to medication timely, accurate and simple. Through our pharmaceutical contractor and our plans for an in-house COMPASS pharmacy, we make medication access convenient and hassle-free. Clients who cannot afford their HIV medication can ask about our Medication Assistance Program, which provides financial help to assure that all clients receive the medications they need.

COMPASS Center Pharmacy

COMPASS pharmacy specializes in HIV care and is dedicated to meeting all of your medication needs. Our pharmacy provide the most advanced HIV medication along with outstanding personal support to help you succeed with HIV treatment.

Our pharmacy is staffed by HIV experts who provide on-going counseling to help you manage and adhere to your medication and resolve any problems that may pop up. The pharmacy works with North Central Health District and other HOPE Center medical providers to ensure that your medication needs are met and your questions are answered.

When you choose COMPASS pharmacy to fill your prescriptions, you’re also choosing t help others with HIV. All funds earned at the COMPASS pharmacy are used to support and/or expand services to those already served by HOPE Center.

Contracted Pharmaceutical Services

Curanthealth has been contracted by HOPE Center and serves clients’ pharmacy needs via mail delivery. Medications can be delivered at no cost to your home, or to the HOPE Center for pick up. Those who have insurance and have co-pays can sign up for pharmacy services at Curanthealth and HOPE Center will cover you co-pay cost. Individuals who have prescriptions from other doctors can also add these to their list of medication and receive all their medication in one delivery.

Housing & Transportation Services

Having secure housing is highly beneficial for you HIV treatment. With safe and affordable housing, you can focus on your treatment plan, adhere to your medications and improve your health. Similarly, having access to reliable transportation can make all the difference in you ability to see your doctors regularly.

Limited Housing Assistance

We offer limited housing assistance through the Housing Opportunities for Persons with AIDS (HOPWA) tenant-based rental assistance program. HOPWA provides housing and case management support for up to 1 year (limited assistance available).

Housing Case Management

Case management services are provided by our housing specialists who work with you to create a housing action plan. Together you will explore your best housing option, provide housing or shelter service referrals and assist with applications for programs outside HOPE Center.

Transportation Services

HOPE Center offers transportation to medical, dental, mental health and support group services at the center and within the community. HOPE Center maintains its own vehicles and provides non-Medicaid transportation services throughout our 13-county coverage area.

Insurance and Financial Assistance

Insurance

HOPE Center accepts multiple insurance plans and will work with your insurance provider to ensure comprehensive care coverage.

State AIDS Drug Assistance Program (ADAP)

ADAP provides access to HIV medication and treatment services for uninsured individuals who meet certain income guidelines. We can also assist with prescriptions that may not be covered by ADAP.

Health Insurance Continuation Program (HICP)

HICP assists those who have active insurance and meet specified income guidelines, but who are unable to pay their health insurance premiums. The program pays monthly health insurance up to $1,375, which includes a spouse and/or children on a family health insurance plan, as well as dental and vision. HICP is available to Georgia residents who are enrolled through District Part B Health Department enrollment sites, of which HOPE Center is a participant.

Client Fees

HOPE Center service costs use sliding fee scale based on individual income and Federal Poverty Guidelines.
